America's Most Popular Prescription Drugs

A narcotic painkiller tops Forbes' list of the most prescribed medicines.

By Matthew Herper, Forbes

Find more

The most popular medicine in the U.S. was prescribed 128 million times last year, even as a panel of experts called together by the Food and Drug Administration recommended that regulators ban it.

The drug is Vicodin, a 40-year-old addictive medicine that combines the narcotic hydrocodone with acetaminophen, the active ingredient in Tylenol; the prescription tally also includes numerous generic versions. It is emblematic of the trend that emerges from our look at the most popular prescription pills: the death of the blockbuster drug.

Forbes' list of the 15 most popular drugs comes from IMS Health, a company that tracks sales at the pharmacy level for drug companies. The list shows how medicine makers are rapidly losing their grip on the average consumer. Only one drug in the top 15, Pfizer's Lipitor, is a big-selling brand-name medication. The rest are cheap generic versions of one-time big sellers that have lost their patent protection and become commodities. Generic copies of Vicodin go for 30 cents a pill, compared to $4 per pill for Lipitor.

Vicodin is a drug that is crying out to be replaced. Vicodin has a supporting role on the television show House, M.D. because the protagonist, a Sherlock Holmes-like doctor, is hooked on it. It is particularly dangerous because when patients develop a tolerance for the narcotic they start taking too much. This can lead to liver failure from overdoses of the acetaminophen component of the combo. Most of the 400 deaths due to acetaminophen poisoning each year result from misuse of the Vicodin or similar drugs. Last June an FDA advisory panel recommended banning Vicodin. The FDA is still deciding whether to take this advice.

"People are taking this as if they're taking a strong Tylenol," says Michel Dubois, director of pain medicine at New York University Langone Medical Center. There's nothing wrong with Vicodin after a surgery or tooth extraction, he says, but it's being used too often by primary care physicians with "minimal training" in dealing with chronic pain. "It has been trivialized, and it's wrong to trivialize this strong a pain medication," he says.

The failure to replace Vicodin is not for lack of trying. Pfizer once hoped its painkiller Bextra could be an alternative. But in 2005 Bextra, a chemical cousin of Merck's Vioxx pill, was pulled from the market because it increased the risk of heart attacks and allergic skin reactions. Pfizer paid a $1.2 billion criminal fine last year for marketing Bextra for unapproved uses like dental pain. Other efforts to replace Vicodin have come up short. Vicodin-maker Abbott Laboratories worked for years on a delayed release formulation but has not been able to get it through the FDA.

The second most popular drug in the U.S. is generic Zocor, or simvastatin. The cholesterol-lowering drug was once Merck's biggest seller, but Merck lost its patent protection in 2006. Doctors prescribed it 83 million times last year. One reason: It is proven to save lives, and it costs only $1 a pill, far less than Lipitor.

Zocor used to play second fiddle to Lipitor but became more popular after it went generic and health insurers pushed it as a cheaper option. Merck, Pfizer and others tried to develop new types of cholesterol drugs with little success; the cholesterol drug market is now dominated by generics.

Lipitor still wracks up $7.5 billion in annual sales, more than any other drug but only ranks seventh by popularity, with 51.1 million prescriptions last year, down from 75 million in 2005. The second-biggest seller by dollar sales is the heartburn drug Nexium from AstraZeneca. But it is a lackluster No. 19 in popularity. Overall, only eight of the 50 most popular drugs are still branded, compared to 20 in 2003, IMS says; 75 percent of all prescriptions are for generic drugs.

The third most popular drug, with 81.3 million prescriptions, is the blood-pressure-lowering medicine lisinopril, which used to be sold under the brand names Zestril and Prinivil. Overall, six of the top 15 most prescribed meds are blood-pressure-lowering drugs. There are so many good generic blood pressure drugs that it is difficult for brand-name drugmakers to improve upon them.

The No. 5 drug, azithromycin, is the generic version of Pfizer's popular antibiotic Zithromax. It tallied 53 million prescriptions last year. Like Vicodin, it's a much maligned medicine whose popularity was once blamed on overmarketing. It's not being advertised to doctors or consumers anymore, but it's still overused.

Brand name drug companies are inventing fewer drugs—a third as many were approved in 2009 as in 1999—and the ones they are inventing aren't selling. The last good year for new medicines was in 2006. Drugs launched that year generated $1.2 billion in first-year sales, says IMS. In none of the years since have drugmakers come close to this figure.

Companies are responding by trying to focus on medicines they can sell to small numbers of people at a higher price for rarer diseases like cancer, multiple sclerosis and ultra-rare genetic diseases. Ironically, that may include the next big painkiller: an injected protein drug that blocks nerve growth. It is being developed by Pfizer and could hit the market in a couple years.

The Top 5:

  1. Hydrocodone/acetaminophen (painkiller)
  2. Simvastatin (high cholesterol)
  3. Lisinopril (high blood pressure)
  4. Levothyroxine (thyroid disorders)
  5. Azithromycin (antibiotic)

Eat Like Skinny Women

By Megan McMorris

We all have that one thin friend -- one of those skinny women who's never bullied into submission by the bread basket, and when she says, "I'll just eat one bite," she does just that. Is she for real?

Turns out, research shows that skinny people simply don't think about food the same way as, well, the rest of us. "Thin people have a relaxed relationship with food," explains David L. Katz, M.D., an associate professor adjunct in public health at Yale University. "Those who are overweight, however, tend to be preoccupied by it. They focus on how much or how often they eat, or attach labels like "good" and "bad" to certain foods. As a result, mealtime is always on the brain."

Here, weight-loss experts explore the mysterious minds of the "naturally" slim, and how to eat like skinny women. Learn what they do, what they don't, and how you can act the part.

They choose satisfied over stuffed

On a fullness scale of 1 to 10, skinny women stop eating at a level of 6 or 7, says Jill Fleming, R.D., author of "Thin People Don't Clean Their Plates." The rest of us may keep going to an 8 or 10. Why? It may be because you mistakenly equate the sensation of fullness with satisfaction and feel deprived if you stop short, says Fleming. Or you may just be used to finishing what's in front of you, regardless of whether you really need it.

Copy them: To eat like skinny women, about halfway through your next meal, put your fork down and, using the 1-to-10 scale, rate your level of fullness. Do it again when you have about five bites left. The goal is to increase your awareness of how satisfied you feel during a meal. (Bonus: It also slows down your eating, which allows the sensation of fullness to settle in.)

They realize hunger isn't an emergency

Most of us who struggle with extra pounds tend to view hunger as a condition that needs to be cured -- and fast, says Judith S. Beck, Ph.D., author of the book "The Beck Diet Solution." "If you fear hunger, you might routinely overeat to avoid it," she says. Thin people tolerate it because they know hunger pangs always come and go, buying them some time.

Copy them: Pick a busy day to purposely delay lunch by an hour or two. Or try skipping an afternoon snack one day. You'll see that you can still function just fine. Then next time you feel those grumbles, you'll hold off before making a beeline for the fridge.

They don't use food to cure the blues
It's not that skinny women are immune to emotional eating, says Kara Gallagher, Ph.D., a weight-loss expert based in Louisville, Kentucky. But they tend to recognize when they're doing it and stop.

Copy them: Add the word "halt" to your vocabulary, says Gallagher. More than just a command (as in stop eating that entire sleeve of cookies), it's an acronym that stands for Hungry, Angry, Lonely, or Tired -- the four most common triggers for emotional eating.

If you're truly hungry, eat a balanced snack, such as a handful of nuts, to tide you over until your next meal. But if you're angry, lonely, or tired, seek an alternative, calorie-free solution to your emotional need. Blow off steam by going for a run or just jumping around - the heartbeat boost will help dissipate your anger. Lonely? Call a friend, e-mail your kid, or walk to the park or mall. Being around others will make you feel more connected to your community (even if you don't bump into anyone you know). If you're tired, for heaven's sake, sleep!

They eat more fruit
Skinny women, on average, have one more serving of fruit and eat more fiber and less fat per day than overweight people, reports a 2006 study published in the Journal of the American Dietetic Association.

Copy them: Start tinkering. To eat like skinny women, examine your diet for ways to add whole fruits (not juices) to your meals and snacks. Aim for two or three servings per day. Sprinkle berries in your cereal or on your yogurt. Add sliced pears to your turkey sandwich, or bake an apple for dessert. Keep a bowl of fruit on your kitchen table or desk to motivate you to think fruit first, vending machine never.

They're creatures of habit
Any dietitian will tell you that a varied diet is good -- but too much variety can backfire, says Katz, author of "The Flavor Point Diet." Studies have shown that too many tastes and textures encourage you to overeat, he explains. "Thin people have what I call a food groove -- the majority of their meals consist of well-planned staples," says Beck. "There are a few surprises thrown in, but for the most part their diets are fairly predictable."

Copy them: Try to eat as consistently as possible with your major meals -- have cereal for breakfast, a salad at lunch, and so forth. It's OK to add grilled chicken to the salad one day and tuna the next, but by sticking to a loosely prescribed meal schedule, you limit the opportunities to overindulge.

They have a self-control gene

Researchers at Tufts University found that the biggest predictor of weight gain among women in their 50s and 60s was their level of disinhibition, or unrestrained behavior. Women with low disinhibition (in other words, a finely tuned sense of restraint) had the lowest body mass index. High disinhibition (i.e., low restraint) was linked to an adult weight gain of as much as 33 pounds.

Copy them: Prepare for moments when your disinhibition is likely to be higher -- such as when you're in a festive atmosphere with a large group of friends. If you're at a party, tell yourself you'll take one of every fourth passed hors d'oeuvre. If you're out at dinner, order an appetizer portion and share dessert. Or if you're stressed -- another low-restraint moment -- make sure you have a source of crunchy snacks (like fruit or carrot sticks) at the ready.

They're movers and shakers
On average, skinny women are on their feet an extra 2 1/2 hours per day -- which can help burn off 33 pounds a year, according to a study from the Mayo Clinic in Rochester, Minnesota.

Copy them: Try a reality check. Studies have shown that people often overestimate how active they really are, says Gallagher. Most people actually spend 16 to 20 hours a day just sitting. Wear a pedometer on an average day, and see how close you get to the recommended 10,000 steps. Your day should combine 30 minutes of structured exercise with a variety of healthy habits, such as taking the stairs instead of the elevator or mopping the floor with extra vigor.

They sleep -- well
Skinny women snooze two more hours per week compared with overweight people, says a study from Eastern Virginia Medical School. Researchers theorize that a lack of shut-eye is linked to lower levels of appetite-suppressing hormones like leptin and higher levels of the appetite-boosting hormone ghrelin.

Copy them: Break it down: Two extra hours of sleep a week is only 17 more minutes a day -- a lot more manageable, even for the most packed of schedules. Start there and slowly work toward eight hours of snooze time a night -- the right amount for most adults.

Quick Tip: Eat breakfast! Seventy-right percent of successful dieters do it every day, according to the National Weight Control Registry, a database of more than 5,000 people who've lost more than 30 pounds and kept the weight off for at least a year.

脸谱 A

Keep fit

Selina

Keep fit

Keep fit

Keep fit

Chubby people live longest: Japan study

Fri Jun 19, 6:14 AM
TOKYO (AFP) - Health experts have long warned of the risk of obesity, but a new Japanese study warns that being very skinny is even more dangerous, and that slightly chubby people live longer.

People who are a little overweight at age 40 live six to seven years longer than very thin people, whose average life expectancy was shorter by some five years than that of obese people, the study found.
"We found skinny people run the highest risk," said Shinichi Kuriyama, an associate professor at Tohoku University's Graduate School of Medicine who worked on the long-term study of middle-aged and elderly people.
"We had expected thin people would show the shortest life expectancy but didn't expect the difference to be this large," he told AFP by telephone.
The study was conducted by a health ministry team led by Tohoku University professor Ichiro Tsuji and covered 50,000 people between the ages of 40 and 79 over 12 years in the northern Japanese prefecture of Miyagi.
"There had been an argument that thin people's lives are short because many of them are sick or smoke. But the difference was almost unchanged even when we eliminated these factors," Kuriyama said.
Main reasons for the shorter lifespans of skinny people were believed to include their heightened vulnerability to diseases such as pneumonia and the fragility of their blood vessels, he said.
But Kuriyama warned he was not recommending people eat as much as they want.
"It's better that thin people try to gain normal weight, but we doubt it's good for people of normal physique to put on more fat," he said.
The study divided people into four weight classes at age 40 according to their body mass index, or BMI, calculated by dividing a person's weight in kilograms by their squared height in metres.
The normal range is 18.5 to 25, with thinness defined as under 18.5. A BMI of 25 to 30 was classed as slightly overweight and an index above 30 as obese.

A Better Life -wish you well: news for healthy living

We're moving on, but we won't be far away

Good morning,

On this day in the year 73, the Masada fell to the Romans. In 1799, Napoleon drove the Ottoman Turks across the River Jordan. Charlie Chaplin was born on April 16, 1889 and Marie Tussaud, founder of the famous wax museum, died on this date in 1850.

And this date marks the swan song for the Better Life blog, which is being discontinued in a reorganization of the site. The reporters whose work you've seen here, including Rita Rubin, Liz Szabo, Nanci Hellmich, Mary Brophy Marcus, Sharon Jayson, Michelle Healy and Steve Sternberg, will still be filing stories to the newspaper and to health.usatoday.com. Anne Godlasky, the online producer who wrote many items herself, will still be creating dynamic online presentations.

So don't touch that dial. Check out the Health and Behavior page. Look up your favorite reporters on the reporter index and you'll be directed to their stories. Or follow @USATODAYhealth on Twitter for the latest news and tips that we find around the Web. Don't forget to take this poll telling us which health issues you're most interested in (you can vote for more than one topic).

Some famous people are known for their last words. Humphrey Bogart was reported to have said, "I should never have switched from Scotch to martinis." Henry VIII is said to have exclaimed, "All is lost! Monks! Monks! Monks!" Neither seems quite appropriate for this moment, although each has its unique charms.

Perhaps the best sign-off is the phrase that has been part of this blog's title: "Wishing you well." Which we do.
